Manual toothbrushes should have a permanent handle and replaceable screw-on brush heads, the same way a razor has a permanent handle and replaceable blades.
The permanent handle could be made of durable plastic or metal. The heads would still be of plastic — but when it was time to replace the head, there would be 75% or 80% less plastic waste.
Make the heads recyclable, and you could potentially eliminate toothbrush waste altogether.
You could buy multi-packs of brush heads in different colors, designs, themes, bristle styles, etc.
You could even buy other tool heads, like a dental pick or a mirror attachment to help with oral hygiene in other ways.
